This is 24/7 one-stop center is designed for referring physicians. Access Center staff will route your call while you wait. You will always speak to a live person who will make sure your call is taken.

Use this toll-free number to: get professional consultation with a specific physician or a physician in a specific specialty or subspecialty; facilitate direct physician-to-physician patient admissions; arrange emergency air or ground patient transport; facilitate the referral of a patient to Children's; or remain informed about a referred patient's condition and/or discharge plans. If you wish to speak to a physician who is unavailable, an Access Center representative will get you to an available specialist or arrange for the requested physician to return your call that day.

Outpatient Referral Form

The Outpatient Referral Form can be filled out on your computer and e-mailed to Children's Hospital, printed for your records, or saved to your own computer.

To e-mail the form: You will see an "e-mail form" button in the upper right corner. Make sure you are online and at a computer that has your e-mail software loaded. Pushing the button will create an e-mail with an address already in the send field and the completed form attached. Make sure you complete the form before pushing the e-mail button. You can then either save or print the form for your own records. Also, please include your name and phone number in the body of the e-mail so that our staff can contact you if they have questions.

Very Important! If you have patient records to send with the referral or any diagnostic orders or court orders, you must FAX the referral form with all attached paperwork. Please do not Fax materials separate from the form or create duplicate submissions (i.e. don't fax and e-mail the form).

To FAX the form: You can fill the form out on your computer, print and fax it to (559)353-8888.

You can also keep using the Outpatient Referral Form as you always have. The electronic version of the form is simply meant to give you a more streamlined option.

Maternal Fetal - Prenatal Diagnostic Physician Referral Form

The Maternal Fetal - Prenatal Diagnostic Physician Referral Form can be filled out on your computer and e-mailed to Children's Hospital, printed for your records, or saved to your own computer.

To E-mail form: Selecting the "email" button in the upper right corner will prompt you to choose between "Desktop email" (email software program loaded on your computer such as Outlook) or "Internet email" (email program through the internet such as Yahoo). The Desktop option will create an e-mail for you with our address already in the send field and the referral form attached. Select "Send". If you choose the Internet option, it will prompt you to save the completed form. Open a new email page through your email provider and attach the form. Send to MFCReferral@ChildrensCentralCal.org

Very Important! We need the Patient Prenatal Records. If you have the electronic records you may email them to MFCReferral@ChildrensCentralCal.org along with the referral or you can fax the records to (559) 353-6710.

To FAX form: Once you have filled out the form on your computer, select "Print". Fax the printed referral to (559) 353-6710.
